TERMS

Customer agrees to surrender all owned satellite equipment (receiver(s) and remote control(s)) in their possession to BendBroadband. Customer will indemnify, defend and hold BendBroadband harmless from any outstanding obligations they may have pertaining to their satellite television service or the surrendered equipment. If the equipment is leased, customer must supply a contract or current bill reflecting the lease agreement at the time of execution of this agreement. Customer agrees to a 12-month television services contract, with a minimum of Essentials or Preferred digital cable service, commencing from the point of service activation. Cable TV services over and above Essentials or Preferred level are not subject to the 12-month service agreement.

BendBroadband will provide free standard installation of one outlet of the video services subscribed to at the time of this agreement (may also include HDTV and DVR). High-speed Internet service installation will be charged at the current price, taking into account any promotional pricing at the time service is ordered. BendBroadband will provide a $200.00 credit towards cable television service, applied at the rate of $20.00 per month over the first ten months of service. " BendBroadband will provide a free standard AO installation ($29.50 value) and a free DCT700 digital box ($1.95/mo value) for each additional outlet subscribed to at the time of this agreement, when accompanied by the customer's surrender of an additional receiver/remote (or evidence of lease of same). The free DCT700 offer will continue for the 12-month term of this agreement, so long as the customer retains the additional DCT700 on their account. Customers who choose to substitute an advanced set top in lieu of the DCT700 are responsible for the full rental price of those units.

This agreement automatically terminates if the customer moves out of BendBroadband's serviceable area. This agreement will terminate if the customer is disconnected for delinquent/non-payment under BendBroadband policy guidelines. Any unapplied basic service credits are forfeited upon termination prior to the end of the 12-month term. BendBroadband reserves the right to charge back for the accumulated prior month's service credits and $50 installation value in the event of early termination of digital cable service by customer or for non-pay. Surrendered satellite receivers and remotes become the property of BendBroadband and cannot be returned.

A person who has had their service disconnected due to non-payment is not eligible for the Dish Buy Back promotion for a period of 2 years from the date service was terminated.

Dish Buy Back offer may not be combined with any other offer unless expressly stated. A 12 month subscription to at least Digital Gateway service is required. Limited time offer.
